Insurance
We participate in most major insurance plans, including Medicare. As a Medicare participating provider, we accept Medicare’s assignment for claims processing. Please note that we do not participate in Medicaid.
If you are insured through a plan we do not participate with—or if you are paying out of pocket—we offer prompt-pay discounts when services are paid in full at the time of service. If you are unsure whether we participate with your plan, please contact our office and we will be happy to assist you.
If we are in-network with your plan, your coverage and benefits are determined by your insurance company. While we will review your benefits and provide an estimate of costs during check-in, it is ultimately your responsibility to understand your insurance coverage. For questions about benefits or claim payments, please reach out directly to your insurance provider.
Payment & Referrals

Co-payments, deductibles, and co-insurance are contractual obligations with your insurance plan and must be paid at the time of service.

If your plan requires a referral, please ensure we receive it before your appointment. Without a referral, you may choose to reschedule or be seen as a self-pay patient after signing a waiver and paying at the time of service.
How We Help Avoid Surprise Bills
We collect your insurance information when scheduling your appointment so we can verify coverage and provide cost estimates ahead of time—helping you avoid unexpected bills. If we have your current insurance information, we will:

Collect your portion at the time of service

Assist in resolving claim issues whenever possible
Your insurance company may need additional information from you directly—please respond promptly to avoid claim delays.
If your insurance coverage changes, notify our office as soon as possible so we can update your account.

Payment Options
We accept Cash, Checks, Credit Cards, and CareCredit for payment.
If your account is over 90 days past due, your account may be sent to our collection agency for review and follow up.
Patient Portal
To ensure we have complete and accurate information for your visit, all patients must complete patient information forms before seeing the doctor. We’ve made this process quick and convenient through our secure Modernizing Medicine Patient Portal.
Once you are registered in our system, you can access the portal using a one-time passcode after confirming your name, date of birth, and email—either through your web browser (best viewed in Chrome) or the APPatient app.
Through the portal, you can:

Access your medical records anytime
Both Dr. Slaughter and Dr. Salvador review patient records and history 24–48 hours prior to your visit. Using the portal ensures your information is received securely and allows our providers to see patients more efficiently during appointments.
If you choose not to use the patient portal, you may arrive 30 minutes early to complete your forms using our in-office kiosk.

Appointments/Cancellations
If you are unable to keep an appointment, please give us at least 24 hours’ notice to avoid a $50 no-show/late cancellation fee. This courtesy allows us to offer your appointment time to another patient in need.
