We participate in most insurance plans including Medicare. As a participating provider with Medicare, we accept their assignment for claims processing. Please be advised that we do not participate with Medicaid. If you are insured with a plan that we do not participate with, payment in full is expected at the time of service. If you are unsure as to our participation with your plan, please contact our office and we can assist you. If we participate with your plan, coverage and benefits are determined by that plan. It is your responsibility to know your insurance benefits. Please contact your insurance company with any questions you may have regarding coverage and how your claims will be paid.
Copayments are a contractual obligation you have with your insurance company and they must be paid at the time of service. If your insurance plan requires that you obtain a referral prior to being seen, it is your responsibility to make sure we receive one, prior to your appointment. If you come to your appointment without a referral you will not be seen by the doctor unless you sign a waiver and pay for your visit.
If we have your current insurance information, we will submit your claims and assist you in any way we can in order to help you get your claims paid. Your insurance company may need you to supply certain information directly to them. It is your responsibility to comply with their request. If you do not supply the requested information in a timely manner, you will be responsible for the balance on your account.
If your insurance coverage changes, please notify our office as soon as possible so that we can make the appropriate changes to your account.

We accept VISA, Mastercard, Discover, AMEX and CareCredit for payment.
If your account is over 90 days past due, your account may be sent to our collection agency for review and follow up.
All patients must complete the patient information forms before seeing the doctor. We have made the process very easy through our patient portal. Once you are registered in our system, our staff will create a user name for you and a password will be emailed to you from Modernizing Medicine. The patient portal is completely secure and works best in the Firefox browser. The patient portal can also be used to email our office staff securely and gives you the ability to access your medical records.
Dr. Slaughter reviews patient records and history 24-48 hours prior to the patient visit. The patient portal makes this process secure for you and allows Dr. Slaughter to see all of his patients in a timely manner when they come into the office.
If, for any reason, you choose not to utilize our patient portal, you may complete the form below. We ask that you fax (512-601-0333) or mail (12221 Renfert Way, Suite 110 Austin, Texas 78758) the paperwork prior to your visit, if possible.
